CAC certificate of incorporation *

The certificate issued by the Corporate Affairs Commission. A clear photo or scan is fine.

CAC status report or Form CO7 (optional)

Showing current directors. Only needed if your certificate is more than five years old.

Proof of business address *

A utility bill, tenancy agreement or bank statement showing the business premises in Imo State.

ID of the contact person *

National ID, driver's licence, voter's card or international passport.

Tax Identification Number (TIN) (optional)

Your FIRS or Imo SIRS TIN certificate, if you have one.

You can submit without every document and send the rest later — your application will simply wait at “awaiting documents” until they arrive.
